Medication Reconciliation

Measure Purpose: To ensure the safety of patients by reviewing and updating the medication list at every visit

The numerator: Medication review conducted at every encounter during the measurement year and the presence of a medication list in the medical record.

  • Patient and/or caregiver is unable or unwilling to do this activity
  • Procedure visit (i.e., EEG, nerve conduction study) where no sedation occurs

Measure Calculation Example

Dr. Wagner saw 100 patients from January 1 to December 31 of the measurement year. This is the denominator. One patient was unwilling to do this activity and three were seen for a procedure. These patients are removed from the denominator.

Of the remaining 96 patients, 55 had a medication review conducted at every visit during the measurement year. This is the numerator.

Quality of care =55/(100-4). Dr. Wagner has a score of 57% for this measure.
