Autoimmune encephalitis has been described as antibodies against neuronal cell surface/synaptic proteins. Autoimmune encephalitis may occur in the presence or absence of cancer and is usually suggestive of inflammatory process. In more than half of patients, the neurologic syndrome develops before the cancer diagnosis is known. Cerebrospinal fluid (CSF) abnormalities include increased protein concentration, pleocytosis, elevated immunoglobulin G (IgG) index, and oligoclonal bands.1

We report a unique case of patient who presented with confusion and shoulder pain with positive ACh Receptor Binding Ab. Magnetic resonance imaging (MRI) showed an inflammatory process in bilateral hippocampus and the in the left brachial plexus.
Patient responded to immunotherapy with resolution of symptoms and repeat imaging showed resolution of findings.

There have been very rare cases of autoimmune syndrome in which both ventral

and peripheral nervous system were involved at the same time.2, 3
