Identifying Factors Pertaining to Migraine-Related Stigma from a Novel Question Set: Results of the OVERCOME (US) study

Disease-related internalized stigma involves an individual’s beliefs about how others view their disease. The current study evaluated the factor structure of belief statements regarding an individual’s beliefs regarding how others view them due to their having migraine. 

Identify factors pertaining to migraine-related internalized stigma derived from a set of questions regarding an individual’s beliefs of how others view them and their migraine. 

Data were pooled from the 2018-2020 cohorts of the OVERCOME study, a web-based survey conducted in a representative US sample (n= 61,935). Respondents with migraine were identified via validated migraine diagnostic screener based on ICHD-3 criteria (95%) and/or a self-reported medical-diagnosis of migraine (58%). The 12-item question set was developed by clinical experts and from patient interviews. The stem for each question was “How often have you felt that others viewed your migraine or severe headache…?” with response options 1 (Never) to 5 (Very Often). Exploratory Factor Analysis (EFA) using an oblique rotation (Promax) was conducted.

The sample had a mean age of 41.7 years, 74% were female, 89% had episodic migraine, and 43% had at least moderate migraine-related disability (MIDAS >11). EFA suggested two factors based on eigenvalue > 1 regarding an individual’s belief regarding how others view them/their migraine. Factor loadings >0.60 were considered significant for inclusion. One factor, termed “Secondary Gain”, consists of 8 questions regarding others believing the individual uses migraine to their advantage (e.g., “get out of…commitments”, “get attention”). The second factor, termed “Minimizing Disease Burden”, consists of 4 questions regarding others’ lacking understanding of migraine’s impact (e.g., “with a lack of understanding…”, “an easily treatable condition”). These factors were consistent when segmenting respondents by monthly headache days or sex.

EFA suggests two factors regarding an individual’s belief of how others view them/their migraine that may serve to assess migraine-related internalized stigma.
