SAkuraBONSAI: A Prospective, Open-label Study of Satralizumab Investigating Novel Imaging, Biomarker, and Clinical Outcomes in Patients with Aquaporin-4-IgG-seropositive (AQP4-IgG+) Neuromyelitis Optica Spectrum Disorder (NMOSD)

Satralizumab, a monoclonal recycling antibody that targets the interleukin-6 receptor, significantly reduced relapse risk with a favorable long-term safety profile vs placebo in two pivotal phase 3 trials. Satralizumab was approved for the treatment of adult AQP4-IgG+ NMOSD patients by the US Food and Drug Administration in 2020.

SAkuraBONSAI is a 2-year, prospective, multicenter, open-label study to assess the efficacy and safety of satralizumab in aquaporin-4-IgG seropositive (AQP4-IgG+) NMOSD patients who are treatment naïve or have failed prior rituximab (RTX) treatment. 

Patients must be 18–74 years of age and have a confirmed diagnosis of AQP4-IgG+ NMOSD (Wingerchuk 2015 criteria). The two study cohorts are: treatment naïve (Cohort 1; n=60); and failed recent (<6 months) RTX or biosimilar treatment (Cohort 2; n=40). Satralizumab monotherapy (120 mg) will be administered subcutaneously at Weeks 0, 2, 4, and Q4W thereafter for a total treatment period of 92 weeks, with a 12-week safety follow-up.

Clinical disease activity measures including time to first relapse, proportion relapse free, annualized relapse rate, and relapse severity will be evaluated over 96 weeks. Disability progression (change in Expanded Disability Status Scale score), patient cognition (Symbol Digit Modalities Test), visual acuity, and patient-reported outcomes will also be assessed. Subclinical disease activity and treatment response will be monitored using advanced optical coherence tomography and magnetic resonance imaging outcomes. The effect of satralizumab on serum and cerebrospinal fluid biomarkers associated with disease pathophysiology, including glial fibrillary acidic protein and neurofilament light chain, will also be evaluated. Safety outcomes include the incidence and severity of adverse events.

This study will provide additional evidence for the use of satralizumab in patients with AQP4-IgG+ NMOSD who are treatment-naïve or responded inadequately to RTX, with insights into disease pathology and progression, treatment response, and the mechanism of action of satralizumab.
