Factors Associated With Seeking Care For Migraine: Results Of The OVERCOME (US) Study

To identify factors significantly associated with seeking care for migraine.

Data were pooled from the 2018-2020 cohorts of the OVERCOME study, a web-based survey conducted in annually recruited demographically representative US samples. Respondents with migraine were identified via a validated migraine diagnostic screener based on ICHD-3 criteria (95%) and/or a self-reported medical diagnosis of migraine (58%). A Random Forest (1000 individual trees) and Least Absolute Shrinkage and Selection Operator (LASSO) logistic regression identified the sociodemographic and disease-related factors most associated with seeking care for migraine. Logistic regression was then conducted among these factors.

A total of 61,829 people with migraine were identified, 46,070 (75%) were female with a mean age of 41.7 years. Among these, 51% (n=31,532) had sought care for migraine in the last 12 months. Among those who sought care, 23,106 (73%) were female and 28,030 (89%) had health insurance. Three factors strongly associated with seeking care in this sample of patients were severe interictal burden (vs none, OR 2.64, CI 2.51-2.77) assessed by the Migraine Interictal Burden Scale (MIBS), severe migraine-related disability (vs little/none, OR 2.16, CI 2.04-2.28) assessed by MIDAS, and severe ictal cutaneous allodynia (vs none, OR 1.70, CI 1.60-1.81) assessed by the Allodynia Symptom Checklist (ASC-12). Severe interictal burden was reported by 53% who sought care and 23% who did not. Severe migraine-related disability was reported by 37% who sought care and 15% who did not. Severe allodynia was reported by 21% who sought care and 7% who did not. 
Migraine-related disability, cutaneous allodynia and interictal burden were strongly associated with seeking care for migraine. It is essential for providers to understand how each patient is impacted by their migraine to optimize management.
