Long-Term Suppression of MRI Disease Activity and Reduction of Global/Regional Volume Loss: Results from OPERA I/II and ORATORIO Open-Label Extension

Efficacy and safety of OCR vs. interferon (IFN) beta-1a in relapsing multiple sclerosis (RMS) and vs. placebo (PBO) in primary progressive multiple sclerosis (PPMS) were demonstrated in OPERA I/II and ORATORIO, respectively.

To assess the effect of switching to/maintaining ocrelizumab (OCR) on MRI measures in the open-label extension (OLE) of OPERA I/II (NCT01247324; NCT01412333) and ORATORIO (NCT01194570).

Unadjusted rate of T1 gadolinium (Gd)-enhancing lesions and new/enlarging (N/E) T2 lesions were analyzed on MRI scans acquired at the 6th (RMS) and 5th year (PPMS) of follow-up after starting the OLE. Percentage changes from study baseline were calculated for whole brain (WB), normal appearing white matter (NAWM), cortical gray matter (CGM), thalamus (THAL), cerebellum (CBL), and T2 and T1 lesion volumes (LV).

In RMS, vs. those who switched to OCR, patients receiving continuous OCR showed no differences in low T1 Gd-enhancing and N/E T2 lesion count (p=0.618 and p=0.760, respectively), less brain structure volume loss and significant/nominal differences in LV accumulation (percentage reduction at the 6th year, OCR-OCR vs IFN-OCR; WB: -6.9%, p=0.070; CGM: -6.2%, p=0.047; NAWM: -8.3%, p=0.240; THAL:-16.9%, p<0.001; CBL: -13.3%, p=0.038; T2 LV: -92.3%, p=0.034; T1 LV: -11.5%, p=0.502). In PPMS, patients initially randomized to OCR showed no difference in T1 Gd-enhancing lesions (p value not available), a small difference in N/E T2 lesions (OCR-OCR vs PBO-OCR rate: 0.010 vs 0.069, p=0.0145), significant/numerical differences in brain volume loss and LV accumulation (percentage reduction at the 5th year, OCR-OCR vs PBO-OCR; WB: -4.9%, p=0.388; CGM: -0.3%, p=0.948; NAWM: -3.1%, p=0.741; THAL: -14.3%, p=0.037; CBL: -9.8%, p=0.259; T2 LV: -70.0%, p<0.001; T1 LV: -28.1%, p=0.022).

In RMS and PPMS, advantages gained by those initially randomized to OCR in long-term reduction of T2 and T1 LV accumulation and global/regional volume loss tended to persist.
