Smoldering disease often manifests clinically in RMS as subtle disability worsening. As we better understand the drivers of disease progression, new players and pathways contained within the CNS are emerging. A key player, the microglia, acts through the CNS immune network to perpetuate the chronic neuroinflammation and neurodegeneration present from the earliest stages of MS.
In this Industry Therapeutic Update from Sanofi, three MS specialists will discuss the nuances of how subtle disability worsening can present, how it can be assessed and tracked in the clinic, and why it is important to patients to identify and label the experience. The specialists will then explore the underlying biology of smoldering disease, the future of disease classification, and developing targeted treatment that addresses the underlying biological processes in the CNS. In particular, the BTK activation pathways in B cells and microglia are discussed as potential targets, with an emphasis on the importance of brain penetrance and bioactivity in the relevant cells involved in the immune network driving MS pathology.
