The Child Neurology Foundation (CNF) has been a leading voice on transition of care issues for more than a decade. Representatives from two grantee academic institutions, Lurie Children's Hospital and Boston Children's Hospital, will share highlights from quality improvement projects their institutions have been tackling for the past year. At this presentation, you will learn both how to implement best practices in transition of care at your institution to improve both patient care, clinical outcomes, and clinician satisfaction, and also how to apply for future grant opportunities in transition of care at the Child Neurology Foundation yourself. This is presented as a joint program with the CNF and AAN.
