Measure Calculation Example

Dr. Oskoui sees 33 patients diagnosed with ALS phenotypes characterized by appropriate ICD Codes or care partners of patients diagnosed with ALS phenotypes characterized by appropriate ICD Codes. This is the denominator. 

There are no exclusions for this measure.

Dr. Oskoui provides 27 of these 33 patients or care partners of patients with ALS information on ALS support services at least once between January 1 and December 31.  This is the numerator.

Quality of patient care = 27/33. Dr. Oskoui has a score of 81.8% for this measure.
