FALLS OUTCOME

Measure Purpose: To record the number of falls a patient reports                                                                                                                      

The numerator: Patients who report a fall occurred during the measurement period

is divided by

The denominator: Patients aged 18 and older with a neurological condition

Exclusions:

  • Patient is bedridden, immobile, not ambulatory
  • No documentation of falls inquiry or discussion during patient visit

Measure Calculation Example

Dr. Mayer saw 100 patients 18 years and older from January 1 to December 31 of the measurement year. This is the denominator. Six of those patients were not ambulatory or bedridden. These patients are removed from the denominator. 

Of the remaining 94 patients, 40 reported a fall occurred during the year. This is the numerator.

Quality of care =40/(100-6). Dr. Mayer has a score of 42% for this measure.

NOTE: A lower score is more desirable for this measure.
